چکیده

Introduction MR first pass perfusion imaging has become a valuable clinical tool for the assessment of myocardial perfusion [1]. The increased spin-lattice relaxation rate 1/T1 and dynamic signal enhancement following the intravenous injection of Gd-DTPA was shown [2] to be a linear function of the local gadolinium concentration. However, studies in rat myocardium [3] revealed that the enhancement may reach a plateau, which would inhibit the deduction of reliable kinetics from observed signal intensity-time curves. The aim of the present study was to investigate whether such a saturation effect has to be taken into account in MR-based perfusion studies involving multiple injections of Gd-DTPA. The myocardial T1 values were measured at multiple points in time during the study and compared with a reference, i.e. the expected T1 extrapolated from the previously published [4] washout kinetic tables of Gd-DTPA following a single injection.

Cardiac T1 mapping in congenital heart disease: bolus versus infusion protocol for measurement of myocardial extracellular volume

Background Measurement of myocardial extracellular volume fraction (ECV) with T1 mapping cardiac magnetic resonance (CMR) before and after the application of a gadolinium-based extracellular contrast agent enables the assessment of diffuse myocardial fibrosis. The equilibrium between blood and myocardium contrast concentration required for ECV measurements can be achieved with a primed contrast...

Basis for detection of stenosis using venous administration of microbubbles during myocardial contrast echocardiography: bolus or continuous infusion?

OBJECTIVES This study sought to determine the basis of detection of stenosis by myocardial contrast echocardiography using venous administration of microbubbles and to define the relative merits of bolus injection versus continuous infusion. BACKGROUND The degree of video intensity (VI) disparity in myocardial beds supplied by stenosed and normal coronary arteries can be used to quantify sten...

T1 mapping for myocardial extracellular volume measurement by cardiovascular magnetic resonance: bolus only vs primed infusion technique

Background Myocardial ECV can be measured using T1 mapping before and after contrast if the contrast agent distribution between blood:myocardial is at equilibrium. Equilibrium distribution can be achieved with a primed contrast infusion (EQ-CMR), or may be approximated by the dynamic equilibration achieved by delayed post bolus measurement. This bolus only approach is highly attractive but curr...

Bolus administration T1 mapping as a marker of interstitial myocardial fibrosis in severe aortic stenosis

Background Aortic stenosis (AS) is associated with increased interstitial myocardial fibrosis (IMF). This is detectable by cardiovascular magnetic resonance (CMR) after gadolinium infusion. T1-mapping-derived partition coefficients (l) and extracellular volume fractions (Ve) have also been shown to correlate with IMF after a simple bolus administration of gadolinium. We hypothesized that the l ...

Quantitative Myocardial Perfusion Using Conventional Single-Bolus Contrast Imaging Overestimates Absolute Myocardial Blood Flow Compared with Dual-Bolus or Dual-Sequence Cardiac MR Methods

Introduction: Estimates of myocardial blood flow (MBF) from first-pass contrast-enhanced cardiac magnetic resonance (CMR) imaging require accurate measurement of the arterial input function (AIF) from the left-ventricular (LV) blood pool. Both dual-bolus [1] and dual-sequence [2,3] CMR methods have been proposed to maintain the linearity of the LV signal during the contrast passage. Several stu...
